Palpitations are due to consciousness of heart action. As a general rule, whatever causes tachycardia or arrhythmia (irregular rhythm of the heart) can cause palpitations.

CAUSES:

  • Physiological causes: Physical exertion and fever cause tachycardia and increased force of cardiac contraction; palpitations are then some- times experienced.
  • Pharmacological causes: Excessive drinking of tea, coffee or alcohol and smoking commonly cause palpitations. Administration of drugs which increase heart rate can cause palpitations. Examples: ephedrine, atropine, thyroid extract, salbutamol, terbutaline and theophyl- line.
  • Anxiety neurosis: Most cases of palpitations fall in this group and are therefore of no consequence. Strange surroundings, examination hall, interviews may cause tachycardia and palpitations in most people. In neurotics even lesser change of environment may induce the symptom.
  • Cardiac neurosis may be based on the fear of a heart disease, especially ischaemic. A recent death in the family due to myocardial infarction is particularly liable to generate consciousness of heart action, tachycardia and palpitations, and left-sided chest pain.
  • Anaemia: Causes tachycardia and high output of the heart in an effort to compensate for the low oxygen-carrying capacity of blood. This may result in palpitations.
  • Extrasystoles: May be symptomless. Or may produce a fluttering sensation in the heart region and palpitations.
  • Thyrotoxicosis: A fairly common cause of palpitations as it can cause tachycardia, extrasystoles and AF.Look for other signs, viz. loss of weight, exophthalmos, visible sclera above the cornea, lid lag and fine tremors in the fingers of outstretched hands.
  • Hypoglycaemia: Diabetic patients on hypoglycaemic agents. An overdose of insulin or oral hypoglycaemic drugs or skipping of meals may cause the condition. Tachycardia, palpitations, restlessness, sweating.
  • Phaeochromocytoma: A benign tumour of chromaffin tissue arising from the adrenal gland. It secretes vasopressor substances and is the cause of episodic hypertension and palpitations.
