BENIGN TUMORS OF THE VULVA

Fibroma, lipoma, neurofibroma: These are all benign tumors of varying sizes.

Fibroma is the most common benign solid tumor of the vulva. It arises from deeper connective tissues of the labia majora (dermatofibroma). Vulval fibroma grow slowly. It may be small but malignant change is very low.

Hydradenoma: Hydradenoma arises from the sweat gland in the vulva, usually located in the anterior part of the labia majora (38%). It rarely exceeds 1 cm. It is a benign lesion but its reddish look and complex adenomatous pattern on histology may be confused with adenocarcinoma. Simple excision and biopsy is adequate.

Vulval endometriosis is usually found at the site of old obstetric laceration, episiotomy or area of excised Bartholin’s duct cyst. It may appear as a swelling near the insertion of the round ligaments in the anterior part of the labia majora. It may be associated with pelvic endometriosis. Its presence is limited only during reproductive period and becomes enlarged and tender during menstruation.

Vaginal wall Cysts: Vaginal cyst is rare as there is no gland in the mucous coat. Small cysts may remain unnoticed. The common cysts that are found include Gartner’s duct cyst and epithelial inclusion cyst.

Gartner’s cyst:- The Gartner’s duct cyst is usually situated in the anterolateral wall of the vagina. The epithelium is low columnar and secretes mucinous material which is pentup to form the cyst.

Inclusion cyst: Common vulvar cysts are epidermal inclusion cysts or sebaceous cysts. Usually, they are located beneath the epidermis in the anterior half of labia majora. Alternative histogenesis is embryonic remants or occlusion of pilosebaceous ducts of sweat glands. The vaginal epithelial inclusion cysts arise from the epithelium buried under the mucosa during healing from a traumatic injury following childbirth or surgery. They are usually located in the lower-third of the vagina on the posterior or posterolateral wall. The lining epithelium is stratified squamous. The content is a thick cheesy substance.

HOMOEOPATHIC MANAGEMENT OF BENIGN TUMORS OF VULVA:-

  1. Helonias:- There is apthous inflammation of vulva. The parts are red, swollen, itchy. Foul smelling discharge is there.
  2. Kreosotum:- There is violent itching of the vulva especially when urinating. Violent itching between the labias. Swelling and burning of the vulva. Gushing leucorrhoea like water, which is offensive, corrosive, staining linen yellow.
  3. Oophorinum:- There is violent itching of the vulva.
  4. Pulsatilla:- There is burning, itching, in the vulva, labia.
  5. Sepia:- Leucorrhoea with intense itchiness in the genitalia. Leucorrhoea yellow, greenish, milky in large lumps with intense itchiness. Stitching pain up in the vagina from uterus to umbilicus is there.
  6. Thuja:- The vulvas are very sensitive. Itching, burning, and soreness is there. There is thick profuse greenish discharge.
