COR PULMONALE

Cor pulmonale is right heart failure caused by chronic pulmonary arterial hypertension.

CAUSES

Causes include:

• chronic lung disease,
• cystic fibrosis
• bronchiectasis
• interstitial lung disease
• pulmonary vascular disorders, and
• neuro muscular and skeletal diseases.

CLINICAL FEATURES:

Symptoms include dyspnoea, fatique, and syncope

Signs: cyanosis; tachycardia; raised JVP with prominent a and v waves; RV heave; loud P₂ Pap systolic murmur (tricuspid regurgitation); early diastolic Graham Steel murmur, hepatomegaly and oedema.

MANAGEMENT :

Treat underlying cause-eg COPD and pulmonary infections.
